Northway's original description was primarily radiographic. In 1979, the concept of continuing respiratory symptoms and oxygen dependence for at least 28 postnatal days was added (28dyBPD). In 1988, noting that the persistent need for oxygen at 36 weeks postconceptional age (PCA) was more predictive of later pulmonary morbidity, Shennan proposed that oxygen dependence at that age rather than 28 postnatal days be used to define patients with BPD (36wkBPD). This is currently the definition accepted by most neonatologists.

Several scoring systems have been proposed in the literature, both clinical and radiologic, in an attempt to quantify the disease severity. Recently, a national workshop on BPD put forth a new severity-based definition. These efforts recognize that various treatment parameters influence the need for oxygen and the amount required at a specific point in time. Once refined, these newer definitions may allow a more precise prediction of subsequent pulmonary morbidities in premature babies.
